myLaurel is hiring an experienced Registered Nurse to join our care delivery team in New Orleans. You'll be rotating between three Ochsner sites — Baptist, Kenner, and West Bank —to help safely transition patients from hospital to home.
💡 What You’ll Do
🔹 Build relationships with hospital teams to identify eligible patients
🔹 Educate patients/caregivers and support seamless discharges
🔹 Apply clinical judgment to assess eligibility and care needs
🔹 Collaborate with internal teams to ensure smooth handoffs
🔹 Track metrics and drive referral performance
👩️⚕️ Who You Are
✔️ Active RN license in Louisiana (required)
✔️ 2+ years in acute, inpatient, or ED settings
✔️ Skilled in discharge planning & care coordination
✔️ Strong communicator and proactive team player
✔️ Reliable transportation + valid driver’s license
